My God, this is one of the most (if not THE most) cringe-inducing lyrics the man ever penned.

It actually sounds like a very straight-faced parody of a love ballad. In that context, it would be VERY effective. If Weird Al ever decided to get a little more raw, THIS is the kind of style-parody he might've come up with.

Not even passable as a 'guilty pleasure' to my ears.

And then my friend and I had a lovely time cracking all the jokes about scalpels, and how much we'd HATE to have that whole song aimed at us specifically. The worst part? It took a week to get that song out of my head. I was about ready to stick my brain in an autoclave to sterilize it.
