in which he makes reference to violent, anal sex.

You could argue that the general position for anal is doggie. Therefore the headboard would be in front of her head and not behind her head. I always thought this song "turn me loose" is in reference to Mani and his divorce. He uses the words "it's against the law in 13 states what i'm gonna do to you" more like a metaphorical threat rather than physical. So it really has nothing to do with anal sex.
