From what I've heard/read "Sister" supposedly refers to his half-sister who lived in New York in the late 70's. He stayed at hers when he went to New York in 1976 (or 1977) when he tried to get a record deal. Somebody suggested that whatever happened during his stay there was reflected in the song. Although I do believe that it's purely fictional lyrics.
