Does a person know they are dying from drug usage?

I'm still so confused. It seems to me Prince knew he wasn't well, he decided to write his memoirs. He had to know he was ill, does a person feel themselves dying from drug usage? At the same time, he had bought a new piano. If he knew he was dying, why didn't he try to do something about it sooner?
