It could be accidental but it's not common to position someone off the page unless there's a reason so these could very well have been the starting vision.
Especially given the backround and the fact that the dress, though beaded, also reads as chain link. Of course once the hair is done it changes everything. If you crop that off it reads very differently. Also, the parted lips takes away from that...you can almost hear someone off to the side saying "it's not sexy enough...part the lips".

The cover could be a randon snapshot that's about nothing at all...but based on the caliber of photos she puts out, you have to guess that those behind them, have, at least, a starting vision.
