Yes, that's the one and only released "original" cover.
And the album was ALWAYS only called "" Any other names you see like "symbol", "love symbol" or "androgyny" are just unofficial approximations made by fans or stores to give it a title they can print. The actual title has no pronunciation or word equivalent. Theres a few minor variations, as the original CD copies had the large symbol screen printed in gold on the front of the case, not on the booklet. Also, some issues have "Prince and the New Power Generation" written across the bottom in an arabic style font as well. Some copies don't have the parental advisory logo. Also, the European copies had the symbol foil embossed on the CD case rather than using gold ink (which looked even better as it was really metallic). There was also a limited edition version in a shiny gold foil slipcase box with just a purple symbol on the front. However, the cover design was originally going to be plain black, with just the large gold symbol on the front. This was changed a long time before manufacture or release though. Though you are right about the Love Symbol album having no official title other than that symbol, Warner Bros. made the title "Love Symbol" semi-official by calling that album, well, "Love Symbol" on the sleeve of the German Peach single ( the 45rpm vinyl) . It clearly says "from the album LOVE SYMBOL " on the sleeve. Now, that doesn´t mean that Prince ever intended to call it that, and of course it is the artist who has the last word on how we should name his art but I think that printing the name "Love Symbol" on a single cover makes it almost official, doesn´t it ? I´ve looked it up again and yes, it says "taken from the album Love Symbol" but I can´t take a picture right now because my cheapo cam needs to be recharged and the print is to small but I will try again later.It has Peach on the a-side and My Name Is Prince on the b-side. There was just a metal clank sound (like a safe door slamming or something). The same can be heard on the "2 Whom It May Concern" B-side, just once, but it is the same sound effect.
