Condition of the heart was from the era when Prince was involved with Susannah so I always thought it was about her. Another line in this song is 'a woman from the ghetto who made funny faces just like Clara Bow' which I know for a fact was written about Wendy. The line after this is 'how was I to know that she would wear the same cologne as you...' which was a reference to Wendy and Susannah who wore the same perfume.
