I need help with My Avatar

I've tried to make this myself, but I can't succede. I want a Simple Black square, like a CD Jewel Case...with a block print, peach colored BOBGEORGE in the middle. U know, to kind of make it look like the black album. I made it in Word, but I can't get it in a format that will work as an avatar...can someone help me.
