When an erection is abnormally prolonged it is called priapism. It is caused by obstruction of the venous drainage of the penis or trauma causing a high-flow state (like a fistula). Common causes include sickle cell disease in blacks, leukemia in children and intracorporeal injections used to induce erections in patients undergoing treatment for erectile dysfunction. It can also be drug related (heparin, phenothiazines) or idiopathic. The patient often presents quite late because of embarrassment over his situation. At that time there is usually considerable pain due to the ischernia of the erectile tissue. Permanent damage can be done to the penis. Because impotence occurs in up to 50%, priapism is a urological emergency. The first line of treatment is to inject an alpha agonist (0. 1 ml of 1: 1000 epinephrine) directly into the penis. If this is unsuccessful in ending the erection, old stagnant blood is syringed out with an 18 gauge needle and normal saline with or without an alpha agonist is flushed in. More invasive procedures may be necessary if the penis does not decompress with simple irrigation.
