There are a lot of good endings to great movies and I think people are doing an excellent job of listing them. My candidate for best surprise ending is from a terrible movie:

Memento - my brain must have been on vacation that night, because I absolutely did not see Teddy's explanation coming. In retrospect it seems blindingly obvious, of course - and it can be interpreted in several ways...
I'd also have to mention the ending of Insomnia (Norwegian version) - not because it was shocking, or came out of nowhere, but because it was so bleak and amoral. | |

I thought Secret Window was too predictable, the only reason I stayed to watch the end was to see Johnny Depp..
Fight Club The Usual Suspects Arlington Road Se7en, I didn't think the head in the box was a surprise but I was surprised when Brad Pitt shot the killer, you sort of expect the cop do the 'right' thing at the end Donnie Darko was just a surprise through out the whole film I was thinking the same thing about American Beauty! I really thought the main character wouldn't reveal the ending in the first minute of the film... | |
