Joni Mitchell is in a coma, TMZ reports.

Per the site, the singer-songwriter, 71, remains unconscious and unresponsive at UCLA Medical Center and that her longtime friend Leslie Morris has filed documents to become Mitchell's conservator. According to documents obtained by TMZ, the Canadian musician is "so impaired as to be incapable of being assessed."

A rep for Mitchell did not immediately respond to PEOPLE's request for comment.

Mitchell was first rushed to the hospital March 31 after she was found unconscious in her L.A. home. The same day, a verified Twitter account for the singer's Tweeted she was in intensive care but "awake and in good spirits."



Reads an April 3 post on the folk star's official website: "Joni remains under observation in the hospital and is resting comfortably. We are encouraged by her progress and she continues to improve and get stronger each day."

The eight-time Grammy winner – known for hits like "Big Yellow Taxi" and Both Sides, Now" – has stated in the past that she has struggled withMorgellons disease, a skin condition which causes sufferers to feel crawling sensations under the skin.

Actually, no she is not OK. The latest news is that there was a court proceeding Monday where a friend of 44 years became her conservator to make decisions regarding her medical needs as Joni is incapable of doing so (this reads to me like she had a stroke... A doctor's report given to the court stated that she was "so impaired as to be incapable of being assessed."

I'm not sure why the Joni Mitchell website is posting that she is fine but it seems they want to present the idea to the world that Joni is fine when she is not. The previous doctor's report also stated that it would be 4-6 months before she could even make decisions for herself.

I've been pretty heartbroken by this very sad news. I also suspect stroke, tumor perhaps, but I am hoping for the best. It's been incredibly frustrating to see so many different reports regarding the state of her health. Why would this friend state that she is alert and they anticipate a full recovry when the report reveals otherwise?

Yes, i think stroke, as well. It may be a misguided attempt at maintaining her privacy, legacy, and calming the concerns of her fans but it's still very bewildering. Also, if the soundbite were true that "she comprehends, she's alert, and she has her full senses" then she wouldn't need someone "to make care decisions" for her for the next 4-6 months.

During a conversation with HuffPost Live about his current solo tour, Crosby offered words of concern and support for Mitchell, who was a staple on the '60s and '70s California music scene, along with Crosby and his collaborators Stephen Stills, Graham Nash and Neil Young.

"I have not spoken to her. To my knowledge, she is not speaking yet," Crosby said of Mitchell. "She is home, she is in care, she is in recovery. How that's going to go, we don't know yet. She took a terrible hit. She had an aneurysm, and nobody found her for a while. And she's going to have to struggle back from it the way you struggle back from a traumatic brain injury."

The most recent official update on Mitchell's condition, posted to her website on April 28, said that at that time, Mitchell was still in the hospital and "she comprehends, she’s alert, and she has her full senses. A full recovery is expected." Crosby said he's pulling for that recovery along with Mitchell's legions of fans.

"I love her. She was my old lady at one point. She's probably the best of us -- probably the greatest living singer-songwriter. ... We're all holding our breath and thinking a good thought, hoping that it's going to turn out okay," he said.
