Absolutely the health part is always an issue. But not buying into society's ideas of beauty and sex appeal is hard for many to do but he's gone overboard exemplifying two different extremes. I'm sure it's just one of a few things he's dealing with at the moment.

He was trying to give the people and the industry what they wanted. He was playing the game but from my understanding he was never really into it or at least comfortable. Some of that led to the break up with Angie (him being attached and to someone who didn't look like a model). So he got ripped, did the nudie video and now... five years later, voila!

It's possible that he did resent the focus on looks/sexuality etc... and it is possible that he did subconsciously become the tubster. Some people are self destructive that way.

Wonder if he'll remake that video in large format. He should do it before Chapelle does.
