do mods have to report it to Ben when there's a ban? and show him why it was done? the reason i ask is say a mod has a personal problem with a user and just decides to ban them or delete them regardless if they have done anything wrong on the org or not

Sure, any org member is free to contact him anytime with regards to any serious and/or unresolved issue that they feel warrants his attention. Although I'm sure I can speak for Ben when I say that any misuse or abuse of this privelage would be heavily frowned upon.
