OK, right I'm a big wrestling fan and ofcourse last sunday was the ECW PPV. Most of the wrestlers there used real songs as their theme music but only one called 'The Sandman' used 'Enter Sandman' as his theme and this was ofcourse the song by Metallica. So I assume WWE licensed just that song. My question is:

Q) Would they have got a license as this other person said it was covered by the venue's performing rights license?

Q) Would they need to pay additional royalties per DVD sold in addition to licensing the song for the PPV?

Q) Why didn't they license the other songs, I presume it would have been far too expensive to license songs by Kiss, Pantera etc. amongst others

Q1: If they didn't get a license, they would be subject to lawsuits by the artists. So yes, if they were smart, they got permission from Metallica.

Q2: If done legally, yes, they would probably have a per unit agreement. But, upon agreement of both parties, they can settle for a one time user fee if they so choose.

Q3: Money is a factor, yes. But in this case there was a theme, so I would imagine that Enter Sandman was a perfect choice for that particular occasion.
If I remember correctly, the Honky Tonk Man used Elvis in his routine. So there are other artists' music used. Professional wrestling has the cash to afford them.

Honky Tonk's gimmick was like an Elvis impersonator but his theme music was written by Jimmy Hart, who was a member of the 50's group The Gentry's and was a wrestling manager, he works for TNA wrestling now.
