There are mainstream movies with full on sex in them, including erections. 9 Songs had unsimulated full-on sex in it. I think it was the same movie where a man ejaculated on camera (masturbation scene). It was the first scene of its kind in a non-porn movie. Shortbus (released 2 years later) had a similar scene in the beginning with a guy doing yoga, and ended up jerking off onto his own face. It was graphic in a different way.
