A three year old doesn't have external testicles though.

Testicles don't descend until puberty. There is nothing but a sack of skin beneath his penis.

With respect, that may have been your experience, but it certainly wasn't mine. My testicles were present in my scrotum for as long as I can remember having a penis (...which was about the same time I realized I had fingers and toes). The only change that happened as I approached puberty was that everything grew larger.

Typically, if a boys testicles haven't descended from his body by the time he's born, there should be cause for concern.
