It means that Vienna is overall a clean city with very clean air, but it is not yet clean enough when it comes to dog shit. They don't "admit", they simply mention.

That was in a time when the streets were polluted with other things and diseases (often spread because of fecal matter, for example) were all around.

People live in cities, because they prefer cities to having contact with animals. If you want to live in dog shit, move to the country side! It's as simple as that.
