Because subcutaneous fat is in a single layer that's composed of only fat. It doesn't have nerves or a blood supply, and it sits on top of, and separate from, muscle tissue.

I don't know exactly how you make sure you're only getting that adipose (fat) layer when you're doing it yourself on your bed, though.
