I have, a few times.

My mom was revived from clinical death 11 times. She described her experiences to me. Kinda cool.

I've never been lying in a hospital bed, but I once got into a major car accident where it was 3am, raining, back country roads, so no light but my headlights and as I rounded a bend there was a car coming straight at me in my lane. I was able to pull far enough onto the shoulder that he didn't hit me head on, but sideswiped me. The window shattered and I had glass all over me. Totaled my car, but all I got were a few cuts on my arm. He was unharmed.
I haven't owned a car since.

many times, yes.
Been in 2 accidents.
Been in the army, got hit by a molotove bottle.
what else?...hmmm...hit my head 3 different times falling on a metal radiator.
Fell off 2nd floor.
I could go on and on...

Yes... a few times:

I had an allergic reaction to Sulfa which left me hospitalized in critical condition for weeks.

I almost died giving birth to my daughter... she almost died as well.

I got into a horrible auto accident, thankfully everyone was okay, with minor injuries.

I nearly got hit by a speeding car while walking through a parking lot... they came VERY close.

I nearly died from a drug overdose when I was a teen and being stupid dabbling with them in the first place.

There are other instances, but I think this is enough.

Just a couple times:
I was in a car accident where the car that hit me(perpendicularly going about 60mph) struck me two foot from my front door. Totally demolished my backdoor.

Another time, i was riding home in the pouring rain on my mountain bike and i was almost run over by a Jeep commanche. The truck/suv(?) stopped at a stop sign, saw me but gunned it and i quickly jammed my brakes, skid, and slid sideways toward the truck. I let the bike go and the truck rolled over it, while i luckily stopped 2 ft from the truck's wheels.
They purposely kept going for about 40 ft from me, slowed down, and then took off. It was so rainy out, i could have easily been dead and they wouldn't have known it.
I then picked up my potato chip for a bike and dragged it home.

I've been in 1 scary car acident. The car was spining out of control, crossing over to the other side of the road... thankfully there wasn't much trafic.

One really scary thing that happened to me once...

We were hiking and forgot to bring water on a hot day. There wasn't much shade, kind of a canyon-type area. I started getting dehydrated and suddenly, while standing at a lookout point, my knees got so weak I couldn't stand up anymore. I didn't faint, just couldn't see for a few seconds and I felt like I had nothing below my knees. It was like I lost my sense of balance and I couldn't feel my feet! Very scary. I gave up and landed on my but. But it really scared me, because not 3 feet away from me was a long way down the canyon. Imagine... Oh God, that freaks me out.

I was heading in2 this store and the parking lot was in the back. As I was walking 2 the front and was 1/2 a step from turning the corner 2 where the entrance was, this suv came whizzing by. What made it so bad was that he was on the sidewalk where he shouldn't have been. It was raining kinda hard and I guess his girlfriend didn't want 2 get wet so he drove under the overhang right in front of the store. All I saw at 1st was a big flash of black. Good thing I have quick reflexes 'cause his grill would've got me right in the ribs.

Probably about five times -- four of those when I was 12-15 years old.

ok its a sob story, but its true!!

0-12 months - hospitalized for pneumonia (my mother swears death was standing in the corner)... i was dehydrating and dying

5yrs - picture it mom in left field softball game - go get a sno cone!!! BAM, in the hospital for 3wks cause i've been poisoned, i am being taped to a bed they are pulling out 5inch needles drilling them into my body - WTH?

6yrs - riding on the back of a motorcyle (yeah) OOps rolling off said motorcyle - busted head wide open on big concrete hill..hospital severe skin damage few nerves gone
26stitches later...
6yrs - sleeping... rolling off the bed (highboy beds) damn hitting the floor - busting stitches open
6yrs - running into the bathroom wham, down she goes cracks head on bathtub!! uuuhhh yeah
7yrs - chicken pox no big deal
7.5yrs - another round of chicken pox and i'm laying in my bed at home with the priest...

18yrs - rolling down beast road, pulling 100mph in the deadliest curve, but i got this cause i have a mission anyway!! right ... seeing something fluttering in the backseat (in my rearview) hair starts that raiseUp...but i can't look - i'm ballin'... i'm pulling down 125mph on one of the deadliest roads in this state, and there is something in the backseat thats making my skin crawl its all in black and i am pulling 130mph... gunning the engine rolling nonstop into the only open spot in this out of the way hick spot and throwing the gears - jumping out of the car running into the store...
when i exited the car the door swung it sort of caught - but when i hit the inside of the store my car door flew open and out scuttles this black shadow type of thing and the leaves starting going bezerk out there and me and the guy working are just jaw open staring...
what u all don't understand was i was looking for death that night and i did indeed find it, but not like that i would/could never kill myself doing something i love - i have paid for not giving the reaper his due...be wise and be careful what u wish for!!

1992 I got stuck on top of a mountain because I was too afraid to ski down, while what looked liked a blizzard was approaching. I was so cold my legs were numb.
Then ski patrol came by & gave me a ride back to the lodge. I was so freakin' scared.
Y wont they ever let u ride the lift back down?!?!?

1998 Was also mugged in on the beach in L.A. by 3 guys. My friend says I had a knife held against my stomach. But all I noticed was a gun on him.

2000 Got in car accident when i was 3 months pregnant. Totaled my car.

2000 Thought I was gonna die during child birth. I nearly passed out from lack of air. Signaled the nurse to give me the oxygen mask & starting shaking uncontrolably.

Also got hit by a car in 90, but it's not worth menthoning. I was crossing a crosswalk with a group of friends by LAX. This car stopped too late & I feel on the hood.

I just realized my story sucks!

almost drowned once but my friend happened to be watching at the exact moment i went under and he swam out and saved me. and ate a cashew...had an anaphalactic(?) seizure was rushed to the hospital just in time...i had just stopped breathing...not much damage though...
