is the "save the women and children first" rule always applicable?

on a sinking ship or a fire or anything like that, the old saying goes that women and children should be saved first, then the men...but should a 60 year old woman be allowed to live while a 25 year old father of four dies? or what about a drug addicted 30 year old woman versus a 40 year old doctor? should circumstances and exceptions be allowed to play a part in the decision making, or is it just "women and children" first?

If there were no women and children only sheep and men then it probably wouldn't apply nod
