A similar event occurred where I live. A successful attorney was having an elective plastics procedure in the Dr's office, respiratory arrested, and couldn't be intubated. She died as a result.
Many plastic surgeries (ie eye lifts, face lifts) are performed in the surgeon's office with a nurse doing conscious sedation (narcotic/sedative).
There are risks. The most important thing is to have emergency equipment up to date, and available. Including reversal agents.
Some offices use nurse anesthetists and some use RN's.

It's unfortunate this happened, and I feel bad for her son.
