I think that's due to the angle - it necessitates the need to rub someones head but not look at them. Kinda like rubbing your head and stomach at the same time.

I emoticons! I do try to use them in real life. I was just sending an email to a friend of mine and I was looking all over for the emoticon. I couldn't find it.
