It depends on the meat but usually medium is preferable. Definitely not rare; I don't like that slightly gelatinous texture (or rubbery texture as the case may be if the quality of the meat is not as good).

It always makes me laugh when some guys fire off a big smug grin when they order it rare, as if they want to prove how tough they are or something.

I already said that; the ones that order it just because they think it proves how tough they are. Such people do exist unfortunately, and like Charlotte said it's the same kind of guys who order extra hot curry etc. A bit silly if you ask me.
