I've found San Francisco is a good place for high end lingere. French and Italian brands. I have a black and white pokka dot bra and thong with little bows, and a black sequined set with multi colored sequins which is great under lacy tops. The sequins are secure and don't catch. Recently bought a Betsy Johnson pink with black pokka dot set a couple of weeks ago.

A friend of mine had it done and she nearly lost her nipple to necrosis...the blood supply to the nipples (since they have to be removed) can be comprimised and that is a big risk.

Actually the enlargement is a much less invasive surgery...they usually make a small bellybutton incision and then insert everything through that.

First off, you need to find a good plastic surgeon. There's some who specialize in breast surgery for cancer patients, implants, and reductions and have more experience.
Secondly, it depends on the health of the patient.(Immunosuppressed, poor hygiene,ie.. not washing hands with home dressing changes,etc) Nipple necrosis is not common.

so don't let that scare you away
if you want a reduction the best thing
to do is go for a consultation.
