In many of the profiles under Gender, i've seen the answer "Not sure".
It seems like a simple enough question. Your sex based on plumbing.

Is it just an overused joke or am I missing something here?

Maybe some folks don't wish to define "who" or what they are by such narrow definitions, because certain expectations go along with those definitions.

Also regardless of how one may identify intrinsically or emotionally as a different gender..., which is their right, that doesn't change the plumbing which technically defines gender.

I think the fact that there is the choice of *not sure* as a selection is just too tempting for some people to click it as a "joke". Not explaining myself real well, but that's how I took it when I saw it was people would select it trying to be funny...ya know?

When you have to use a "non-unisex" public restroom, which door do you enter?
Male or Female? (maybe that's what the Profile page should ask)

It you're part of the Gender: Not sure crowd (non joking variety), I would imagine you've got a major problem.
Ultimately, your bladder is going to force you to make a choice.
