If we're just talking about a still photo: It's all about the eyes. They make or break the entire mood of the subject, the pose, the entire composition.
If we're talking about a live person or a moving image: Attitude says it all. The way a person carries themself, and how they regard themselves and others. They don't even have to be a pleasant person, it's in how the whole package fits together that makes it sexy or not. For example: Mick Jagger's looking pretty weathered and a bit spooky these days, but he's got a fucking hot attitude. Therefore, I think the dude's sexy.
