the hymen for one thing. also for some woman not having sex for a while can cause pain when they start to do it again. after my wife had the twins and after being off limits to lovin for 15 weeks (bed rest and recovery) it hurt her for the first 3 or 4 times... so i would think that if your a virgin and thus had not used it for 18 years it would hurt just due to using those mussels for first time.
